Description
This Business Associate Agreement (BAA) template is designed for use by a Covered Entity entering
into an agreement with a Business Associate, in accordance with the Health Insurance Portability
and Accountability Act of 1996 (HIPAA), as amended by the Health Information Technology for Economic and Clinical
Health (HITECH) Act.
This document has been updated to incorporate the requirements of the HITECH Act and is provided in Microsoft Word format for ease of use. Minimal customization is required, including:
- Inserting the legal name of the Covered Entity (the purchaser of this template),
- Inserting the legal name of the Business Associate (the third party with whom the Covered Entity is contracting),
- Providing appropriate contact information for both parties.
