This Business Associate Agreement (BAA) template is intended for use by a Business Associate to establish a HIPAA-compliant agreement with its subcontractors (subsequent Business Associates). It is not intended for use by a Covered Entity to enter into an agreement with a primary Business Associate.
This template has been updated to reflect the requirements of the Health Information Technology for Economic and Clinical Health (HITECH) Act and is provided in Microsoft Word format for ease of customization.

- Inserting the legal name of the initial Business Associate (the purchaser of this template)
